Purpose
Infectious disease occurs worldwide and must be addressed just as chronic disease is approached. This assignment will offer the opportunity to explore the various communicable diseases, the epidemiological background data as well as the implications of these infections.

This paper should clearly and comprehensively identify the chronic health disease chosen. Select a topic from the following list:

The paper should be organized into the following sections:
- Introduction with a clear presentation of the problem as well as significance and a scholarly overview of the paper.
- Background of the disease including definition, description, signs and symptoms, and current incidence and/or prevalence statistics by city/state with comparison to national statistics pertaining to the disease. Create a table of incidence or prevalence rates by your geographic county/city or state with comparison to national statistics. Use the APA text for formatting guidelines.
- A review of current surveillance methods and any mandated reporting or methods for reporting the disease for providers.
- Conduct descriptive epidemiology analysis of the disease including who is more frequently affected and characteristics of the population that might help in creating a prevention plan. Include costs (both financial and social) associated with the disease or problem.
- Review how the disease is diagnosed, current national standards for screening or prevention, and pick one screening test and review its sensitivity, specificity, positive predictive value, cost and any current national guidelines for conducting which patients to conduct this test on.
- Integrating evidence, provide a plan of how you will address this chronic health disease in your practice once you are finished with school. Provide three specific interventions based on the evidence and include how you will measure outcomes of your actions. Note: Consider primary, secondary, and tertiary interventions as well as integration of health policy advocacy efforts.

Purpose
The concepts of epidemiology provide the framework for the study of infectious and chronic diseases, which provides a rich source of data for the analysis of trends in disease and health. This assignment will offer the opportunity to explore the health effects associated with hookahs. You will identify populations at risk, and to design an educational intervention for your target population. In addition, you will look at what outcomes will be addressed to determine if your interventions work. You will integrate HP2020 and CDC information into your paper.

Research the health effects of hookahs and select three to five health effects to address. Identify your target population (for instance, age or other demographic, aggregate population); this must be in your city or state. Design a population-based health education intervention for your target population that is aimed at reducing morbidity and mortality for the problem. Be sure to review the research literature and HP2020 for interventions. Decide what outcomes you will use to determine whether your educational intervention works. In a three (3) page summary, address the following.
- Identify the problem in the introduction section.
- Provide an overview of the problem in your city/state
- Review of descriptive epidemiologic and demographic data on mortality/morbidity and risk
- Present the goal, overview and objectives of Healthy People 2020 for the paper topic.

Find one health policy enacted within the last two (2) years.
Write a minimum of a one-page critical analysis summary of the policy. (250 words double spaced, APA). Your summary should integrate ANA ethical statements (“The Code”), and course readings, to include a minimum of one course scholarly article (provided within the course). Be sure to speak to the role of advocacy and population health.
