The purpose of assignments Part 1 – Part 3 is to gradually guide the student in developing the signature assignment. The idea is for the student to take a stepwise approach to completing the signature assignment. The signature assignment will be broken up into three steps:

Introduction
Heart failure is recognized as one of the most important core measures by the Centers for Medicare and Medicaid Services (CMS). Based on the data from the Centers for
Disease Control and Prevention (2019), there are about 6.5 million adults who are living with heart failure across the country. American Heart Association (2016) also indicated that heart failure affects 5% of the population who are 65 and older in the United States.
Furthermore, healthcare organizations have already spent approximately $30.7 billion in 2012 in Heart Failure treatment and maintenance, and the costs will double the amount in just twenty years by prediction (Usinowicz et al., 2020). Thus, to reduce the readmission rate for Heart Failure patients, especially for the ones who are age 65 and older will tremendously decrease the healthcare costs and improve care quality for the senior citizens.
Overview Of The Problem
It is not uncommon that patients return to hospitals within 30 days of their discharge.
The report that Medicare published by Gorina et al., (2015) showed that 17 % of the Medicare patients who are 65 and over were readmitted to the hospital within 30 days after discharge, and Heart Failure cases make up 24 % of the readmissions.
Unfortunately, patients who are age 65 and older with readmissions most likely will suffer from some serious medical complications due to prolonged hospital stays. It also costs a vast amount of financial stress for both the patients and healthcare organizations. Therefore, post-discharge follow-up programs are developed by hospitals to reduce the readmission rate for heart failure.
A collaborative effort from the multidisciplinary team facilitates the transition to home for the patients on discharge. The goal for these programs is to improve the quality of life, medication reconciliation, self-care skills, and coordination of care. Age-specific goals are taken into considerations for senior patients in the programs, such as assessment of the readiness, need for the second learner, and alternative education tools.

The student will select an approved topic from the list (or with faculty approval as previously stated) and will customize the question to meet his or her interest.

- Provide a title that conveys or describes the assignment.
- Introduction – Provide an introduction to your topic or project. The introduction gives the reader an accurate, concrete understanding of what the project will cover and what can be gained from implementation of this project.
- Overview of the Problem – Provide a synopsis of the problem and some indication of why the problem is worth exploring or what contribution the proposed project is apt to make to practice.

The paper for Week 3 should be 3 to 5 pages not including the title page or references. An abstract is not required and will neither add to or subtract from the grade for inclusion or exclusion.
