**Discuss your management plan for this patient, including pharmacologic therapies, tests, patient education, referrals, and follow-ups.

Ruptured TOA is a life-threatening potential complication and a surgical emergency. Surgical drainage of the abscess via interventional radiology or laparoscopy may decrease treatment time and directly obtained cultures may provide antibiotic guidance (Silva et al., 2015). All patients with a high suspicion for a TOA need to be hospitalized and intravenous antibiotics immediately started.

 According to the Centers for Disease Control and prevention broad spectrum intravenous antibiotics including coverage for N. gonorrhoeae and C. trachomatis should be started empirically for the first 48 hours to 72 hours which include: Ampicillin/Sulbactam 3 g IV every 6 hours and Doxycycline 100 mg orally or IV every 12 hours. Referral to an Obgyn for follow-up which is usually seen three days after hospital discharge and then again one to two weeks later to confirm that the infection has cleared CDC, 2015). Patient education

 ■ Instruct patients to take all of their medications, if they feel nauseated, and to contact their provider promptly if they experience vomiting or are unable to take their medications.

■ Instruct patients to have sexual partners get treatment if they had sexual contact during the 60 days preceding the patient’s onset of symptoms. Treat empirically for both chlamydia and gonorrhea.

■ Advise patients to avoid sexual contact until the infection has been cured.

■ Provide education about sexual risk reduction, using condoms with every sexual contact to prevent becoming reinfected and to prevent other STDs.

 What laboratory studies (if any) should be ordered to help establish the diagnosis? How should the results be interpreted?

A urine pregnancy test should be performed in any woman of reproductive age with adnexal tenderness. If the pregnancy test is positive, then a beta subunit of human chorionic gonadotropin (β-hCG) level needs to be obtained.. A complete blood count with a differential may show an elevated WBC count with a left shift. If the patient is febrile or hemodynamically unstable, then blood and urine cultures need to be ordered. In addition, tests for sexually transmitted diseases including chlamydia and gonorrhea should also be obtained during the pelvic exam.
