Assessment Description

Describe the steps of the evidence-based research process and the importance of using them. In the initial stages of an EBP project, where do nurses in your specialty go to locate sources of information that help them to determine whether or not a practice problem is appropriate for an evidence-based practice change proposal. Include two specific sources on information in your discussion.

The PICO or PICOT method guides the evidence-based research process. A clinical question is converted into a searchable format for locating the most relevant evidence using this method. The PICOT method begins with determining the population and the condition or disease in question. The next step is to decide which interventions or variables you want to study in relation to your population and condition or disease. The third step is to determine what you are comparing your intervention to, followed by determining what improved outcome you are seeking. The “T” step of PICOT is not required for all evidence-based research, but it is for the time period you are looking at for the research studies (DeNisco, 2021).

Once the PICOT question is developed, you begin to search databases for the best and most current evidence. The answers to the PICOT questions form your key words you will use to search for research evidence. Once you have gathered the evidence, you will then critically appraise the evidence. You will formulate recommendations based on the strength and quality of the most relevant and best evidence. This evidence is integrated with clinical expertise and your patients’ goals and values to inform your practice change. The next step is to evaluate the outcomes of the change in practice, followed by dissemination of the practice change in your facility or group (DeNisco, 2021).

To begin researching a practice problem, I would search for sources of evidence in databases. One database relevant for Healthcare Quality and Patient Safety is the Agency for Healthcare Research and Quality (AHRQ). This is a free database that includes evidenced-base practice, outcomes and effectiveness, technology assessments, and clinical practice guidelines. CINAHL Complete is a comprehensive research database for nursing and allied health, which requires a subscription (DeNisco, 2021).

Evidence-based practice (EBP) occurs when research, clinical expertise, and patient values and preferences all come together to guide safe patient care (Horntvedt et al., 2018). Assess, ask, acquire, appraise, apply, and evaluate are the six steps in the research formulation process (EBP, 2022). Begin with the patient to determine or “assess” the clinical problem. Ask a clinical question based on the problem you evaluated. Obtain the necessary resources to conduct a database search. Examine the research’s validity and applicability in practice. Apply the evidence to the patient and assess the patient’s outcome (EBP, 2022).

When asking a clinical question, you could apply the PICOT method to provide a structed search question. PICOT stands for: patient population; intervention; comparison intervention; outcome and time (DeNisco, 2021).

When starting to locate sources of information for an EBP proposal one search bank is the Agency for Healthcare Research and Quality (AHRQ). The AHRQ is designed to make health care safer and more accessible and works with the U.S. Department of Health and Human Services to make sure that evidence is understood and used (AHRQ website, n.d.).
